All I can say is by contributing and being apart of a community to hold you accountable (as well as having friends/family around) is what continues to make yourself successful.  Don't be afraid to go to them and say, "Look, you know and I know I've had an issue with my weight, if it gets out of hand, I give you permission to say something to me."&lt;/p&gt;&lt;p&gt;Screw the social norms, this is about your life. My surgery is in 2 weeks, and I "fear" sliding back, also. I have friends who have been very successful, and others that have been very successful at defeating the surgery.
